Your Rights as a Patient
At Utah Valley Eye Center, we believe every patient deserves to be treated with dignity, respect, compassion, and professionalism. We are committed to providing high-quality eye care while making sure you understand your care, your choices, and your rights as a patient.
We encourage you to ask questions, participate in decisions about your care, and speak up whenever you have concerns.

As a patient of Utah Valley Eye Center, you have the right to:
Be Treated With Respect
You have the right to receive care in a respectful, considerate, and non-discriminatory environment, with recognition of your personal dignity, privacy, and individuality.
Receive Information About Your Care
You have the right to receive information about your diagnosis, recommended treatment, available alternatives, potential risks and benefits, and expected outcomes to the extent appropriate and permitted by law.
Participate in Your Health Care Decisions
You have the right to participate in decisions regarding your eye care and to ask questions about any recommended treatment, procedure, medication, or service.
Ask Questions
You are encouraged to ask questions about your condition, treatment options, costs, insurance coverage, or anything else related to your care. Our team will make reasonable efforts to provide information that helps you make informed decisions.
Refuse Treatment
You generally have the right to refuse recommended treatment or services, to the extent permitted by law. Our providers will explain the potential medical consequences of refusing recommended care.
Access Your Medical Information
You have rights under applicable federal and state law, including HIPAA, to access and obtain copies of your protected health information, subject to applicable legal limitations.
Request Changes to Your Medical Record
You may request an amendment to your medical information if you believe it is incorrect or incomplete. Requests must be made in accordance with our applicable procedures.
Request Privacy and Confidential Communications
You may request that we communicate with you about your health information in a particular manner or at a particular location when permitted by law.
Request Restrictions
You may ask us to restrict certain uses or disclosures of your protected health information. We are not required to agree to every requested restriction, except where required by applicable law.
Receive Information About Our Privacy Practices
You have the right to receive a copy of our Notice of Privacy Practices and to ask questions about how we use and disclose your protected health information.
File a Complaint
If you believe your rights have been violated or you have concerns about the care or services you received, you have the right to raise those concerns without fear of retaliation.

You may also submit a complaint to the U.S. Department of Health and Human Services Office for Civil Rights. Filing a complaint with our office or with the Office for Civil Rights will not result in retaliation.
